- Verbally  - please call 07 3073 1412 and ask to speak to Leesa (CEO)

- Print the below complaint form and post it to "Unit 28, 120 Bloomfield Street, Cleveland, 4169"

- Print the below complaint form and email it to leesa.walters@appliedabilities.com.au 

- Complete the online complaint form.
